Service requests
In order to receive the service, an application shall be submitted to the municipality of Riga State, which shall specify the name, registration number, legal address of the legal person, or the given name, surname, birth data, the location of the installation of public entertainment equipment and the time from the beginning of construction to the end of construction, the name of the entertainment equipment and registration number in the register of dangerous equipment.
The following documents must be attached to the application:1. Project and layout plan for the installation of public amusement equipment (the project must include the arrangement of the public amusement equipment area, access and approach roads, fencing layout, passenger boarding area arrangement, lighting, placement of warning and information signs; the layout plan must indicate the placement of public amusement equipment and specify the location of service structures and the control panel of the amusement equipment);
2. Technical passports of public amusement equipment (scanned in .pdf format);
3. Approval or agreement with the owner or legal possessor of the real estate on which the land is located where the public amusement equipment will be installed (except when the equipment is planned to be placed on land owned or possessed by Riga City Municipality);
4. Document confirming civil liability insurance (policy) and a document confirming its payment (payment order);
5. Document confirming the right to represent a legal entity, if representation is not based on the statutes.
